Tournament Waivers
RISE Indoor Sports requires everyone who enters the premises to sign a waiver relinquishing RISE of any liabilities regarding injury or COVID-19 exposure. This applies to players and spectators.
No one will be allowed into a tournament without a waiver on file.
Player covid-19 waiver
This waiver is for anyone who is playing In a tournament at rise.
Spectator covid-19 waiver
This waiver is for anyone coming to rise to watch a tournament.
